In order to attend Calhoun City Schools, NEW students and their families must complete the following process.
Step 1. Registration for NEW students
Parents/guardians must attend Central Registration between July 20-31, Monday-Friday from 10:00 a.m.-6:00 p.m. The family will register all of their school-aged children during this one visit. The following is a list of required documents that must be submitted.
o 1. Picture I.D. of Parent/Guardian or person enrolling student:
a. Georgia Driver’s license
b. Government-issued identification card
c. Work identification card
d. Other approved identification card
o 2. Documentation of Child’s Date of Birth
a. Certified copy of a birth certificate or certified hospital issued birth record;
b. Military ID card or certificate of live birth from abroad from State Department;
c. An adoption record;
d. A valid passport;
e. A religious record signed by an authorized religious official;
o 3. Social Security Card or signed waiver
o 4. Georgia Certificate of Immunization (Georgia DHR Form 3231)
o 5. Georgia Eye, Ear, and Dental Exams Certificate (Georgia DHR Form 3300)
o 6. Proof of Residence. (Current is defined as within the previous 30 days)
a. The person enrolling the student shall provide proof of residency.
One of the following items with the physical address of the residence must be presented

Home ownership title (warranty deed, quit-claim deed, or security deed);

Lease or rental agreement with name, address, and/or telephone number of lessor;

Current utility (gas, power, water, etc.)

Current government agency mail (food stamp/ Medicaid/ SSI statement);

Current employer documentation (paycheck stub, health insurance statement);
-
Current bank or financial institution statement;
-
Current property tax statement;
-
Current homeowner’s insurance policy;
b. If the person enrolling the student is not the responsible occupant of the residence, the following items are required for proof of residence.
i. The Statement of Legal Residence must be signed and notarized by the parent/legal guardian and the residence owner/lessee. If parent/guardian is enrolling more than one student in the district, each student should be listed on form and copies provided to each school; AND
ii. The residence owner/lessee must provide one of the documents from the above list in item (a).
o 7. Previous School Information (unless entering Kindergarten):
a. Withdrawal form from previous school; OR
b. Name, address, phone and fax number of previous school.
The following documentation shall be presented if applicable:
o 8. If person enrolling student is not the biological parent, legal proof of guardianship is needed (court
documentation or DFCS documents). If guardianship documentation is not provided, student shall be
provisionally enrolled for 30 days in order for person enrolling student to obtain necessary court/government documents and a Non-Parental Affidavit will be necessary. If court/government documents are not presented within 30 days, student may be withdrawn from school. Grandparents without legal guaradianship may provide a Grandparent Power of Attorney form.
o 9. Any court orders or legal documents that prevent or limit access of a parent to the child or child’s educational records.
o 10. Non-Resident Contract
*If the above student qualifies as homeless in accordance with McKinney-Vento Law, the above documentation is not required for enrollment, but as much documentation as available is appreciated.
Step 2. Registration Forms
The following forms will also need to be completed during registration. We encourage you to print and complete these documents prior to your arrival at the Central Registration office. If you are unable to do so, copies will be provided for you to complete during your appointment. For Registration Forms in Spanish go to "Registration Forms" tab.
Step 3. Enrollment
Once registration is completed at the Central Registration, parents/guardians and students will visit the assigned school to enroll and obtain student handbook, class assignments, schedules, and other information unique to the school. The family MUST present the packet of forms listed above. Otherwise, student cannot be enrolled.
