Registration for the 2012-2013 school year will be held


Monday, July 30 through Friday, August 3 from 8:00 a.m. to 6:00 p.m.

Parents of NEW students, please click here for a list of documents needed for registration.
Parents of RETURNING students who did NOT pre-register, please bring picture ID and two proofs of residence. The proofs of residence must be current and have the physical address listed.  Only the following documents will acceptable:
• Home ownership title (warranty deed, quit-claim deed, or security deed);
• Lease or rental agreement with date, name, address, and/or telephone number of lessor;
• Current gas, electric, or water bill (within 30 days);
• Current government agency mail such as food stamp, Medicaid, or SSI statement (within 30 days);
• Current employer documentation such as paycheck stub or health insurance statement (within 30 days);
• Current property tax statement; or
• Current homeowner’s insurance policy.
If the person enrolling the student is not the lessor or owner of the residence, the following items are required for proof of residence.
• A Statement of Legal Residence (available on website under Enrollment/Registration) must be signed and notarized by the parent/legal guardian and the residence owner/lessee.  If parent/guardian is enrolling more than one student in the district, each student should be listed on form and copies provided to each school; AND
• The residence owner/lessee must then provide two of the documents from the above listed acceptable proofs.
***If a Statement of Legal Residence is submitted, additional proof may be requested at the beginning of the school year.

Below is a list of forms that may be used during the registration/enrollment process.  Forms marked “Required” are necessary for any student enrolling in Calhoun City Schools.  Forms designated “Optional” are not required for every student.  There is a brief description of each “optional form” to guide parents in determining if they will need that form as well.  Forms marked with an * are available in Spanish and can be viewed by clicking the "en espanol" link.


All parents/guardians enrolling a student in CCS will also need to submit two proofs of residence as documented on the Enrollment/Registration Home page.

If student is transferring from another school, this form gives permission for CCS to request those records.

Non-Resident Contract*

Statement of Legal Residence*

When an enrolling student is living with another family and do not have any utility or lease documentation in their parent/guardian’s name.

Non-Parent Affidavit

When a child is living/staying with someone who is not their legal custodian and does not qualify as homeless.  This form is good for 30-45 days until said adult can obtain some type of legal custody of child.  *If child is homeless or in foster care, form is not necessary.

Grandparent Power of Attorney


When a student is residing with a grandparent but grandparent does not have legal custody.  Parent should sign document giving grandparent authority to enroll child in school.

SSN Objection Form*

Padres de estudiantes que REGRESAN, traigan por favor un ID con dos pruebas de residencia que estén al día. Las pruebas de residencia deben ser actuales y tener la dirección física enlistada. Sólo los siguientes documentos serán aceptables:
• Título de propiedad (escritura de garantía, para dejar de reclamar título de propiedad o seguridad de escritura);
• Contrato de alquiler con fecha, nombre, dirección y / o número de teléfono del arrendador;
• Factura actual de servicios públicos de gas, luz o de agua (dentro de 30 días);
• Correo actual de agencias de gobierno, tales como estampillas de comida, Medicaid, declaración de SSI (dentro de 30 días);
• Documentación actual del empleador, tales como recibo de sueldo o declaración de seguro de salud (dentro de 30 días);
• Declaración actual de impuestos sobre la propiedad, o
• La póliza actual de seguro de propiedad.
Si la persona que inscribe al estudiante no es el arrendador o propietario de la residencia, los siguientes elementos son necesarios para la prueba de residencia.
• Una Declaración de Residencia Legal (disponible en el sitio web bajo la solicitud de inscripción) debe ser firmada y notariada por el padre / tutor legal y el propietario de la residencia / arrendatario. Si el padre o tutor inscribe más de un estudiante en el distrito, cada estudiante debe aparecer en forma y copias deben ser presentados a cada escuela; Y
• El propietario de residencia / arrendatario, debe proporcionar dos de los documentos de las pruebas aceptables mencionadas arriba.
***Si se presenta una Declaración de Residencia legal, una prueba adicional puede ser solicitada al inicio del año escolar.
