2019-2020 Applications

PREK REGISTRATION | 2019-2020 Calhoun PreK & Junior Jacket Academy Registration

Calhoun City Schools will begin accepting applications for Pre-Kindergarten and our Junior Jacket Academy Programs for the 2019-2020 school year on January 22, 2019. All applications and registration documents can be located at www.calhounschools.org/pre-k-and-jr-jacket-academy or can be collected from our Central Office at 380 Barrett Rd, Calhoun, GA 30701. All PreK applications must be returned by February 1, 2019 to be considered for the PreK Lottery. In late February, school officials will conduct a lottery to fill our available classrooms. Parents will be notified by mail in March if their child has been chosen to attend Calhoun PreK during the 2019-2020 school year. All Junior Jacket Academy Applications will be accepted on a first come basis

All PreK Applications must include completed enrollment forms and COPIES of the following documents:

  1. Birth certificate (Children must be 4 years old on or before September 1st, 2019)

  2. Social Security Card

  3. Immunization Form (#3231) No other form will be accepted.

  4. EAR, EYE & DENTAl Form (#3300)

  5. Proof of ONE of the following if child receives services:
    TANF
    Medicaid
    Peachcare
    SSI
    Food Stamps
    Child and Parent Services (CAPS)

All Junior Jacket Academy Applications must include the following:

  1. Completed Enrollment Form
    Calhoun City Schools in its enrollment procedures, requirements arid processes does not discriminate based on race, color, national origin or immigration status.

  2. Birth Certificate for the child (will need copy of original)

  3. Custody Documentation (.if applicable)
    If applicable, a parent must provide satisfactory documentation of a relationship with the student to be registered. Examples of satisfactory documentation include the following:
    Divorce Decree
    Adoptive parent -official adoption decree
    Guardian = original official guardianship decree
    Foster parent-official foster parent letter

  4. Student's Social· Security Card (will need copy)

  5. Certificate of Immunizations -Form 3231 This form must have a future date of expiration (It must be current)

  6. 2 Self-Addressed, Stamped Envelopes (for notifications mailed to you)
