To ensure your Pre-K Application is complete, please include the following:

Birth certificate (Children must be 4 years old on or before September 1st, 2024)
Student's Social Security Card
Immunization Form (#3231) *No other form will be accepted
Ear, Eye & Dental Form (#3300): All four parts must be completed with a "passing" status. Partial completion is not acceptable. If any part is not "passing," provide documentation of a follow-up appointment or a note from the physician indicating evaluation for the issue. Incomplete forms will be returned for proper completion.
If the student has not undergone their 4-year well child exam at the time of application, include a copy of an appointment card from their physician indicating the exam date. The completed form must be submitted to the Pre-K office within 90 days from the first day of school, as per State of Georgia requirement.
Proof of Georgia Residency (acceptable documents: current lease, current vehicle registration form, letter from shelter, letter from employer if housing is provided, mortgage documents, property tax notice, homeowner's insurance bill, or current Peachcare eligibility documents along with any utility bill indicating the residence as the service address. Cell phone bills or driver’s licenses are not acceptable proof of residency).
Proof of ONE of the following if the child receives services: TANF, Medicaid/PeachCare, SSI, food stamps, Child and Parent Services (CAPS).

Ensure all necessary documents are included with your Pre-K Application to complete the enrollment process.
