REQUIRED VACCINE FORMS FOR 7th & 11th GRADERS | Per Georgia state law, all 7th and 11th-grade students must submit an updated Form 3231 (Immunization Certificate) to the CMS/CHS campus school nurse by September 17, 2025.

✅ Make sure your child’s vaccines are up to date by scheduling an appointment with their doctor or the Gordon County Health Department at (706) 624-1444.
